As the needs of individuals and families cross numerous traditional service boundaries Axis Health System is bringing care for the entire person to one location. Our goal is to provide comprehensive and integrated services in one setting. Beginning as Southwest Colorado Mental Health Center, we changed the name under which we are doing business to Axis Health System in April of 2010 to reflect the transformation and changing role of our Center from exclusively mental health and substance use services to an integrated health system merging our traditional behavioral health services with primary care, wellness and health lifestyle services.
Axis is in partnership with, and supports integrated services in, key regional primary care resources including the Cortez Rural Health Clinic (operated by Southwest Memorial Hospital), the Dove Creek Health Center, the Durango High School school-based health center, and Pediatric Partners primary care practice in Durango. We will be opening a fully integrated healthcare practice in Cortez in the summer of 2011. Stay tuned!
We are also applying to establish a Federally Qualified Health Center (community health center) in La Plata County. This is not an easy process so please let us know if you want to help! |

Axis Health System is a private, not-for -profit corporation. Founded as Southwest Colorado Mental Health Center in April of 1960, we have been providing services to the five counties of southwest Colorado (Archuleta, Dolores, La Plata, Montezuma, and San Juan) for 50 years! |
